camp waiver & release

Every participant at camp must complete a Waiver and Release form. You will need your group’s Registration ID (Reg ID) to complete this form. The Group Leader from your church or ministry can provide this for you. The waiver will pre-populate with your submitted information, as well as your specific group and camp information. Complete the additional required information, sign, and submit!

To obtain a waiver from our signing partner, Airslate, click the button below, complete the required fields, and then choose one of three delivery methods (listed below). From the time you request the form (sent to email or phone), you have two hours to complete and sign it. After two hours, if it is not signed, the form will be deleted automatically, and you will need to request a new waiver.

Instructions for Completing Form Fields

Once you click the button below, you will have several fields of information to complete. All fields are required:

  • Registration ID: This will come from your Group Leader.
  • Participant First Name: Enter the legal first name of attendee.
  • Participant Preferred Name: Preferred name or nickname. This will be used for their nametag.
  • Participant Last Name: Enter the legal last name for the attendee.
  • Gender: Male or Female
  • Participant Grade: Grade completed for the 2025/2026 school year.
  • Track Selections: You will have the opportunity to indicate Track Time preferences for your camper. To learn more about track options, click here.

Once you have received your custom link to Airslate, complete all additional required and applicable fields, sign, and submit.

Request a waiver & release form

HOW TO SIGN THE WAIVER
Check the box and click submit. This will create a link (valid for two hours) to complete and sign the waiver immediately.


Links are valid for two hours. If you are unable to complete the form within that time frame, you can request another.

Troubleshooting Tips
  • This form is best viewed on a computer using updated browsers: Chrome, Firefox and Safari.
  • Firewalls, WiFi filters, or VPN’s (at businesses, churches, schools) may prevent this form from functioning correctly.
  • Sometimes opening the link below in an incognito or a private window can be helpful.
  • If you experience an error where the cursor bounces back to the top of the form, click “complete” again.
