Our team is thrilled to be working alongside you as we proclaim the gospel together at CentriKid in 2024! Here are your next steps to begin preparing for the summer. Please pay special attention to due dates for each step. If you have questions at any point, please feel free to reach out to Henry Dutton.

Prep for Camp with the Worship Leader Folder
Step 1: INITIAL Onboarding form & contract
Please take a moment to complete this form. This initial form will allow us to collect and / or verify pertinent information so that we can create and send your contract for camp.
After completion, Amy Cato from our HQ team will initiate your contract in the coming weeks. You will receive the contract via email from “Lifeway Contracts.” Contracts are managed through a third-party service, Concord. Important: When you receive your contract, Concord will prompt you to “Activate your free account.” Please click “no thanks,” and then proceed through the steps to complete and sign. All contracts remain accessible via the unique, secured link provided in the email, so there is no need to create an account.
Complete the onboarding form, and the subsequent contract, as soon as possible.
IMPORTANT DETAILS
In order to set up a contract and process payments, it is very important that you provide consistent information. Our accounting department requires that the name on the contract MUST match the name on your banking information (form W-9 and direct deposit form). If you receive payments as an entity or LLC rather than as an individual, please provide that information for the contract. If you receive payments as an individual, please list the name that matches your banking information. Inconsistent information between the contract and banking information will result in a delay in payment.
In the past, in some circumstances, we have offered to execute separate contracts and payments for individual band members. We are no longer able to offer individual contracts / payments and require each worship leader / band to operate with a single contract and payee. Please contact Henry Dutton with questions.

Step 2: payment information
Due 14 days prior to camp
RETURNING WORSHIP LEADERS
If you’ve been paid by Lifeway/ CentriKid via direct deposit in the past, and if there are no changes to your banking information, no action is necessary.
UPDATES or CHANGES
If you’ve previously been paid by Lifeway/ CentriKid but need to UPDATE your banking information, use the ACH Enrollment Form. Submit the form to [email protected] as directed.
NEW WORSHIP LEADERS
(1) Complete IRS form W-9. Send a completed Form W-9 to Alli Brown. (Alli is part of our CentriKid HQ team and helps with payments).
(2) Complete the ACH Enrollment Form, and submit to Alli Brown.
Important Note: Please remember that the name and contact information you provide on form W-9 must also match the name on your ACH Enrollment Form AND your contract. To avoid potential delays in payment, please be consistent.
Step 3: Backline Request Form (Optional)
Due by April 1
CentriKid Camps does not supply backline (keyboards, drum kits, guitar amps, etc.) for camps within driving distance. In some instances, we may be able to provide backline for flydates upon request. If you need backline for one of your camp dates, please complete the form below prior to April 1. After this date, please contact Henry Dutton for inquiries.
Step 4: Attend the worship leader webinar
Emily Martin, one of our legacy CentriKid worship leaders, will lead a webinar for worship leaders on April 18 at 7:00pm CDT. This webinar and training will cover tons of practical details, such as:
- General vision casting and camp overview
- 2024 service flows
- Guidance on selecting songs
- Tracks and loops
- Adult Gathering
- Personal care and preparation
- and SO much more!

Step 5: band info & BACKGROUND CHECKS
Due by May 15
Worship Leaders, in order to complete background checks and make important preparations for camp (housing, apparel, etc.), we need to know exactly who is coming to camp as part of your group. Please complete this form to provide the required information.
Note: Everyone from your group must pass a national background check prior to arrival on campus, and background checks can take 3-5 business days to process. It is essential that you complete this form in a timely manner.

Dr. Shane Garrison is Professor of Educational Ministries at Campbellsville University School of Theology. Shane has served with CentriKid for ten years as a camp pastor and on the content creation team. He is married to Jennifer and they have two teenage sons.
