step 3: important forms
As in 2022, most of our required forms are now digital. Please do not use downloaded copies of forms from prior years, as these are no longer valid.
Digital forms means there is no longer a notary requirement, and we have taken additional steps in 2023 to make this process easier than ever! When completing forms, you will be asked to provide your Registration ID (Reg ID). This is a six digit number that is provided when you sign up for camp. Providing your Reg ID will pre-populate much of the required information on your forms (group leader contact information, church name and address, and camp week and location)!
You can locate your Reg ID by logging in to your account online. Your Reg ID is also included in the regular “Ramp to Camp” emails from our team. Still can’t find it? Simply email centrikid@lifeway.com, and we can easily look up and provide your Reg ID.
Group leaders will receive a copy of digital forms as they are complete.

Many users on mobile devices (phones, tablets, etc.) are experiencing errors in submitting forms. We apologize for this inconvenience. Please note that camp forms are best viewed and completed on a desktop device using Chrome or Firefox browsers (some versions of Safari may encounter errors).
Camp Waiver and Release
Every participant at camp (kids, team assistants, and adults) must complete a 2023 Camp Waiver and Release form. Be sure to provide your Reg ID when sending this form to your camp participants.

locations with additional forms
If you are attending camp at Linden Valley Conference Center (TN) or Trinity Pines Conference Center (TX), please complete these additional forms.
For Linden Valley, each participant (kids and adults) must complete an additional release form. Bring completed forms and submit at camp. Link below.
Trinity Pines requires a few additional forms. Some are required for all participants (kids and adults), and others only require one form to cover the whole group. Instructions are included. Link below.
(Please remember to bring printed copies of Child Protection Training Certificates to camp at Trinity Pines).
CERTIFICATE OF INSURANCE
Each church group attending CentriKid must submit a Certificate of Insurance (COI).

Send a completed COI to campscoi@lifeway.com prior to your arrival at camp.
Church Agreement Form
Each group must complete and submit one Church Agreement Form. You will need your Registration ID (reg ID) to complete this form.
child protection training
Every adult (and Team Assistants over the age of 18) must complete a Child Protection Training course. Access the training via the link below. You may also share this URL directly with your leaders: https://ministrygrid.lifeway.com/#/redeem-invite/MDVQ/CGND
Group Leaders, please collect training certificates from your group and submit them in a batch. You may email completed certificates to centrikid@lifeway.com. Please include your church name and camp location in the subject. Or if you prefer, you may submit certificates in-person upon arrival.
IMPORTANT: Churches attending camp in Texas must bring physical copies of Child Protection Training certificates to camp.
Additional notes on Child Protection Training: The course is hosted on Ministry Grid, our online training platform. If you already have a LifeWay ID, use those credentials to login. If not, create a new account.
You must watch all videos to completion (even the trailing few seconds at the end) in order to advance to the quiz.
After you complete the course and receive a passing grade on the quiz, you will receive a completion certificate.
This certification is good for 2 years. If you still have a certificate from a prior year that is still valid, you are welcome to turn it in to satisfy this requirement.
Some churches or ministries use MinistrySafe, a third-party service, to provide “Sexual Abuse Awareness Training” for volunteers and staff. If you have completed this MinistrySafe training and can produce your completion certificate, we will also accept this to satisfy our training requirement.
